Inadequate Safety Precaution Causes Fire Accident

Fire accident that occurred at Imperial Sugar Company Plant last year left several important safety messages for us. The company was failed to put adequate precautions against combustible sugar dust hazard, as reported by Chemical Safety Board (CSB) recently. CSB said that the fire accident was entirely preventable as published in nyt.

Taking Notes from T2 Plant Explosion

t2-plant-explosion A runaway reaction that caused explosion at T2 plant on 19 December 2007 had caused four persons died. Cooling pipe blockage or failed valve probably caused the temperature and pressure inside the batch reactor to become uncontrolled and the relief system could not work to save the system.

I am very interested to what Robert Hall, CSB investigation supervisor, said about T2 plant accident as reported by tce recently. It directly deals with how hazard identification or HAZOP study holds a vital role in recognizing uncontrolled reaction risk since process designing phase. (more…)

Learn From BP Texas City Refinery Explosion

Explosion at BP Texas City Refinery on 23 March 2005 that caused 15 people died and hundreds of others were injured was one of the most dangerous plant accidents. CSB had released a video, which shows the accident and the root cause investigation.

Indeed, this petrochemical plant explosion is a very good example when we do not put adequate measures regarding process safety. (more…)

Chemical Plant Accident Record in June 2009

chemical-plant-accident-statistic Since June 2009, I have collected news about accident (incident) related to chemical plant operation and chemical manufacturing. My intention to post about this chemical plant accident statistic is to remind all of us that accident could happen anywhere in plant site due to variety of reasons. So don’t underestimate any unsafe conditions and safety findings that you may find in your workplaces.
